Create Blacklist#

Opens a form to add a blacklist entry.
Blacklist Type - The type of blacklist you are adding.
IP Address - IP Address of a user to blacklist.
Hardware ID - Hardware ID of a user to blacklist.
Region/State - Region/State to blacklist.
Country Code - Country Code to blacklist.
ASN Number - ASN Number to blacklist.
Enter blacklist data - The data you are blacklisting.
Reason - The reason for blacklisting.

Deletes all blacklisted data.
